Confirmation of change of address
This notification only applies to married couples. In cases where one spouse is moving to a separate legal domicile, the other spouse must approve the transfer on this form within 48 hours of the registration of the transfer notification. If the transfer notification is not confirmed within 48 hours, it will be rejected and a new notification must be submitted.
